Bullying Reporting Form

This form is for SFPS students, parent/guardians, or staff to report incidents that may be bullying, harassment, or hazing. Once submitted, this report will be sent to the school site administration (principal and/or designee) for follow up and investigation. If preferred, incidents may be reported verbally to the school site principal instead.

Please provide as many details as possible to aid the principal in their investigation into this incident. If behavior has happened multiple times, you will have the opportunity to indicate that.

If you wish to remain anonymous, you may leave any personal identifiers blank.
